SPORTS PHYSICALS SCHEDULED FOR

Sports Participation
In order to participate in a sport, the student needs a valid physical and a signed Parent Permission and Update Form.
Sports physicals are performed in the Health Office prior to the start of the sports season.
Medical Exemption
Students are required by the New York State Education Department to participate in Physical Education classes. The participation in Physical Education is mandatory to meet graduation requirements. If they are unable to participate, a physician’s note with specific medical restriction/exclusion and expected time period of restriction must be presented to the nurse. Students who have a medical exemption are required to have written clearance from a doctor to return to Physical Education/Sports.
School Screenings / New Students
New York State Education law mandates certain school screenings.
Medications
Students may NOT have medications on their person, in their locker or in their athletic bags with the exception of the following:
Students must have a prescription from a doctor on file in the health office for administration of the above along with permission to carry the inhaler or epipen. All other medication including: Tylenol, Advil, Midol, antibiotics, ect. must have a doctor’s order and be kept in health office.
Each student must supply the medication in its original container and properly labeled. The nurse will store all medications and dispense them from the health office.
Physicals
By New York State Law all 10th grade students and students new to the school district must have a Physical Examination.
Physicals May be done in the Health Office or by your family physician. For more information or if you would like to schedule a physical with our Nurse Practitioner, please contact the Health Office.
